Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Registered Nurse (RN) to join our Admissions Team serving Broward County. The schedule is Sunday through Wednesday, 9:00am–7:30pm, supporting patients and families through one of life’s transitions with empathy and dignity.

Responsibilities include evaluating referrals, educating families on hospice care, and coordinating individualized plans of care with physicians and liaisons.

Qualifications

• RN licensed in Florida required.
• BSN preferred.
• Hospice certification preferred.
• Current BLS certification required.
• Assess physical and psychosocial needs of patients and families.
• Minimum 1 year in public health, home health, oncology, med/surg, geriatric or hospice nursing.
• Knowledge of hospice nursing, case management, crisis intervention, communication, teaching, thanatology.
• Basic computer skills and EMR experience a plus.
• Adequate transportation with valid Florida driver’s license.

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to join our Admissions Team serving Broward County. This is a meaningful opportunity for an RN who is passionate about guiding patients and families through one of life’s most significant transitions with empathy, clarity, and dignity. You will work the following schedule: Sunday - Wednesday 9:00am - 7:30pm.

What You'll Do

• Serve as the primary nurse responsible for evaluating patients referred for hospice services

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• Possess the ability to assess physical and psychosocial needs of patient and family.
• Minimum of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing, case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
