Trustbridge Hospice, a member of Empath Health, seeks a compassionate Hospice Registered Nurse (RN) Case Manager to join our Broward County team. Monday–Friday 8:30am–5:00pm with weekend coverage; FAQ and full life care focus guides our mission.

Join a not-for-profit network delivering Full Life Care across Florida, with CEU support, tuition assistance, and strong PTO. You’ll collaborate with an interdisciplinary team to ensure dignified, patient-centered care.

Qualifications

• Must be a Florida-licensed Registered Nurse (RN).
• BSN preferred.
• Hospice certification preferred.
• Current BLS certification.
• Experience in hospice, home health, or related nursing preferred.

Responsibilities

• Serve as a key advocate for patients and families, ensuring needs are addressed.
• Coordinate care under physician direction with the IDG team.
• Develop and implement individualized care plans.
• Monitor patient conditions, provide interventions, and educate for comfort and quality of life.
• Address concerns promptly to maintain satisfaction and trust.

Trustbridge Hospice, a member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the north west Broward County area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life’s most meaningful journeys on the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement.

What You'll Do

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
