Job summary

Chapters Health System in Florida seeks a Clinical Team Leader (RN) to oversee daily clinical coordination and staff support for hospice care per the plan of care.

You will provide direct patient care, coordinate IDG activities, and mentor staff while collaborating with Medical Services, Pharmacy, Infusion Program and Continuous Care to address patient needs.

This role requires current RN license, BLS, 2 years nursing experience, EMR/EHR experience, and a flexible, English-speaking schedule.

Qualifications

• Current RN license in the state you work
• Minimum of two years nursing experience
• Experience with EMR/EHR system
• BLS for healthcare professionals from the American Heart Association or Red Cross
• Mobile Driver - Valid driver’s license and automobile insurance per Company policy
• Reliable transportation to meet visit schedule
• Ability to communicate in English
• Flexible schedule to meet the needs of patients
• Ability to physically access homes and perform essential functions

Responsibilities

• Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG) and appropriate nursing intervention
• Coordinates employees and patient care to meet patient and family needs
• Recommends alternate approaches to patient care while following policies
• Educates IDG members on symptom control and resources
• Enhances clinical practice by mentoring staff and serving as clinical resource
• Collaborates with Medical Services, Pharmacy, Infusion Program, and Continuous Care
• Communicates clearly and provides feedback; resolves conflicts
• Ensures timely attention to patient concerns and complaints
• Performs on-site supervisory visits and reviews patient care documentation
• Performs other duties as assigned
