Qualifikationen

• Active RN license in the state of hire.
• 3+ years nursing experience.
• 1 year supervisory/management experience preferred.
• BLS for healthcare professionals from AHA or Red Cross.
• Oncology, hospice or home health experience preferred.
• Geriatric care experience preferred for Nursing Home CMs.
• Valid driver’s license and automobile insurance per company policy.

Aufgaben

• Oversees clinical staff to ensure patient care delivery is efficient and effective 24/7.
• Facilitates/participates in complex patient care conferences to meet care plan outcomes.
• Assist team to determine patients’ clinical needs and level of care.
• Directs weekly IDG meetings when applicable.
• Provides performance feedback and monitors accountability through audits and visits.
• Coordinates care between internal and external parties.
• Administers direct nursing care when appropriate to model clinical competence.
• Manages staffing with the Director per staffing model and budget.
• Collaborates with Pharmacy, Infusion Program, DME, HospiceHelp24 and Admissions to meet care needs.
• Ensures processes support regulatory compliance (recertifications, care plans, orders).
• Participates in QAPI projects and improves performance.
• Address patient/family concerns promptly.
