Job summary

Ethos is seeking an RN Hospice Case Manager to develop referral networks across the Boston market and collaborate with clinical teams to ensure smooth transitions into hospice care. You will educate patients, families and referral sources about hospice services, eligibility and planning, while expanding opportunities and maintaining strong accounts.

Applicants should hold an active RN license and have 1+ year in healthcare sales/liaison; 2+ years is preferred.

Qualifications

• Current, unencumbered RN license required.
• Minimum 1 year in healthcare sales/liaison; 2+ years preferred.
• Hospice/palliative care marketing or liaison experience preferred.
• Boston/South Shore hospital and referral contacts preferred.
• Local market knowledge and proven performance required.

Responsibilities

• Develop and expand strong referral-source relationships across the market.
• Represent hospice services with referral partners, patients, families and community partners.
• Educate referral sources, patients and families on hospice care, eligibility, services and transition planning.
• Identify new referral opportunities and strengthen existing accounts through field activity.
• Collaborate with internal clinical and operations teams to support referrals and smooth transitions.
