Job summary

Optum, part of the UnitedHealth Group family, is seeking a Registered Nurse Case Manager for hospice care. You will oversee the Plan of Care, perform patient evaluations, and ensure high-quality care delivery across home settings.

The role requires an active RN license, reliable transportation, and clinical experience. The position emphasizes collaboration with families and physicians to deliver coordinated hospice services within a patient-centered framework.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• 1+ years of clinical experience.

Responsibilities

• Make the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identify patient/family physical, psychosocial, and environmental needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician, and IDG members in daily care.
