Job summary

Apploi seeks an experienced Clinical Manager to ensure coordinated patient care in a hospice setting. You will supervise clinical staff, review cases, and guide care plans, collaborating with the Clinical Director and interdisciplinary team.

The role requires CA RN licensure (BSN preferred) and 2+ years hospice/home health management; leadership and strong communication are essential.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• Graduate of NLN accredited school of nursing with a BSN degree is preferred or a minimum of one year experience as a professional nurse within the last three years.
• At least two years of experience in hospice/home health or community health setting within the last five years and with two years management or supervisory experience.
• Complies with accepted professional standards and practice.
• Has excellent observation, good nursing judgment and communication skills. Understands hospice philosophy and issues of death/dying.
• Understands principles of pain/symptom management.
• Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements.
• Maintains current CPR certification.
• Demonstrates excellent observation, verbal and written communication skills.

Responsibilities

• Receives case referrals and assigns hospice personnel; consults with Medical Director on eligibility for services.
• Reviews and evaluates cases to promote effective performance and quality home care services.
• Reviews patient medical information and clinical course.
• Assists clinicians in establishing goals and plans of care.
• Attends case conferences to coordinate care.
• Conducts quarterly record reviews and communicates findings to leadership and staff.
• Assists in screening and interviewing new employees and orientation.
• Contributes to planning and evaluating education programs and organizational policies.
• Promotes adherence to professional standards and customer service orientation.
• Assesses health needs of the community and participates in outreach.
• Participates in quality improvement teams and activities.
• Performs other duties as delegated by leadership.
