Freedom Home Health and Hospice Care Services Inc. is seeking a Clinical Supervisor to ensure coordinated patient care and proper supervision of clinical personnel.

The role emphasizes reviewing cases, guiding clinicians, and maintaining high standards of home care delivery. The successful candidate will possess a current RN license, with BSN preferred, and have experience in hospice/home care and in a supervisory capacity.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• BSN degree preferred or minimum of one year experience as a professional nurse within the last three years.
• Two years of management or supervisory experience in hospice/home care; ability to work in an interdisciplinary setting.

Responsibilities

• Receives case referrals; reviews patient information and assigns hospice personnel.
• Reviews and evaluates each case; guides clinicians to improve performance and delivery of quality home care.
• Reviews patient diagnosis, prognosis, medications, procedures and clinical course.
