Qualifikationen

• Current RN license to practice in the state.
• BSN degree preferred.
• Minimum 1 year professional nursing experience within the last three years.
• Experience in hospice/home care and 2+ years in management or supervisory roles.
• Complies with professional standards and CPR certification.
• Valid driver's license with insured vehicle; good driving record.
• Ability to work in an interdisciplinary team.

Aufgaben

• Ensure patient care is coordinated and managed appropriately.
• Assign hospice personnel to cases and supervise clinicians.
• Lead case conferences with hospice personnel and medical director as needed.
• Conduct quarterly record reviews and report findings to leadership.
• Assist in orientation and interviewing of new employees.
• Develop and evaluate plans of care; set priorities and goals.
• Support quality improvement initiatives and organizational policies.
