Affinity Hospice Management LLC is seeking a Clinical Manager to oversee patient care coordination and supervise clinical staff within a hospice setting. The role involves ensuring services are delivered appropriately and guiding clinicians to promote high-quality home care.

The position requires RN licensure, BSN preferred or recent nursing experience, and 2+ years of hospice/home care supervisory experience. Strong interdisciplinary collaboration and CPR certification are expected.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• BSN preferred or minimum of one year experience as a professional nurse within the last three years.
• Two years management or supervisory experience in hospice/home care.
• Experience in an interdisciplinary setting.
• CPR certification required.
• HCHB and PointCare experience preferred.
• Demonstrates excellent observation and communication skills.

Responsibilities

• Coordinate patient care and supervise clinical personnel.
• Review case referrals and assign hospice personnel to cases.
• Collaborate with Medical Director on eligibility for services.
• Review and evaluate cases through home visits, conferences and chart reviews.
• Manage medical records and EMR workflow.
• Assist in setting immediate and long-term care goals and plans.
• Participate in quarterly record reviews and quality improvement activities.
• Contribute to staff recruitment, onboarding and orientation.
• Assist in education program planning and policy development.
• Ensure compliance with professional standards and practices.
• Engage in community health activities and promote customer service.
• Participate in quality assessment and performance improvement teams.
