Job summary

Advance Hospice Care is seeking a dedicated Patient Care Manager to lead clinical excellence and coordinate comprehensive hospice services. Reporting to the Executive Director or Administrator, you will oversee patient care activities ensuring high-quality, compliant care across the hospice team.

You will oversee scheduling, triage referrals, lead IDG meetings, mentor staff, and ensure regulatory compliance while driving performance improvements at your location.

Qualifications

• Registered Nurse licensed in the state of employment.
• Graduate of an accredited nursing program with licensure.
• Minimum 3 years of direct patient care, incl. 2 years in hospice/home health.
• Knowledge of hospice principles and regulatory standards (Medicare/Medicaid/JCAHO/ACHC).
• Comfort with leading a diverse care team and coordinating care.
• Strong interpersonal, organizational, and leadership skills.
• Competency in clinical documentation and health IT.

Responsibilities

• Oversee and direct patient care to ensure high-quality hospice services.
• Manage visit scheduling and prioritize based on clinical judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead or facilitate Interdisciplinary Group (IDG) meetings and document minutes.
• Serve as mentor and clinical support to associates, supporting performance and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support performance improvement initiatives and operational efficiency.
