Job summary

Gentiva seeks an experienced Hospice Clinical Liaison RN to coordinate intake, assess eligibility, educate families, and facilitate admissions across hospitals and facilities. You will build strong referral relationships and ensure seamless handoffs to the care team.

Requirements include an active RN license in the state, 3–5 years hospice experience (BSN preferred), CPR, and a valid driver’s license. This full-time, on-site role offers competitive pay and a comprehensive benefits package.

Qualifications

• Licensed RN with 3–5 years hospice experience preferred.
• BSN with 5+ years hospice experience preferred.
• Minimum of three years of hospice experience required.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other healthcare professionals.
• Assess patient eligibility for hospice by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families.
• Educate patients and families on hospice care, how it works, and its benefits.
• Complete necessary documentation and coordinate intake and admission to meet legal, clinical, and payer requirements.
• Serve as a liaison between referral sources and the hospice care team for seamless handoffs.
• Make daily visits to hospitals and facilities to stay connected with discharge planners and physicians.
• Build and maintain relationships with referral partners and identify new outreach opportunities.
