Job summary

Magenta Care LLC in Chandler, AZ is seeking a dedicated and experienced home infusion RN to deliver exceptional patient care in the home setting. The role requires travel up to one to two hours outside Chandler to serve clients.

Responsibilities include direct patient care within RN scope, collaborative care planning, start of care through admission, patient advocacy, timely communication of changes, and education on infusion therapy management and self-care.

Qualifications

• AZ RN license or Multistate RN License (Compact) required.
• Minimum of 1 year of recent RN experience (Infusion, ER, ICU).
• Healthcare CPR and Basic Life Support (BLS) certification required.
• Current Arizona Driver's license with proof of insurability.
• Reliable transportation.

Responsibilities

• Provide direct patient care within the RN scope of practice.
• Assess, diagnose, plan, implement and evaluate patient care with active involvement of patients and families.
• Collaborate with physicians and interdisciplinary team to develop a comprehensive plan of care.
• Facilitate start of care through the admission process.
• Advocate for patient rights, safety and preferences; communicate changes promptly to the care team.
• Provide home health nursing care following standards and agency procedures.
• Educate patients and families on infusion therapy management, potential side effects and self-care.

Job description

We are seeking a dedicated and experienced home infusion RN who is passionate about delivering exceptional patient care.

The successful candidate will possess excellent communication skills, empathy, and a strong commitment to maintaining patient privacy and dignity.

The primary location for this position will be in Chandler, AZ, with the requirement to travel up to one to two hours outside of Chandler to serve our clients.

Responsibilities

Provide direct patient care, practicing within the scope of the registered nurse practice act. Continuously assess, use nursing diagnosis, implement, plan, and evaluate patient care, actively involving patients and their families in the process. Collaborate with physicians and other interdisciplinary team members to develop a comprehensive plan of care that addresses the unique needs of each patient and their family. Facilitate patient start of care through the admission process. Act as a patient advocate, ensuring their rights, preferences, and safety are upheld. Document and effectively communicate any changes in the patient's condition promptly to team members, the Director of Clinical Services, and the physician. Provide nursing care to home health patients, adhering to established standards of care and following agency policies and procedures. Educate patients and their families on infusion therapy management, potential side effects, and self-care practices

Requirements

Valid AZ Registered Nurse (RN) license Or Multistate RN License (Compact). Minimum of 1 year of recent RN experience (Infusion, ER, ICU). Excellent IV insertion skills. Healthcare provider CPR and Basic Life Support (BLS) certification. Current Arizona Driver's license with proof of insurability. Reliable transportation
