Job summary

Amedisys is seeking an Admissions Registered Nurse for Home Health in Alabama. The role runs full time from 8am to 5pm, with an orientation base salary of $73,000.00 to $80,000.00.

You will perform start-of-care assessments, coordinate plan of care, and ensure timely OASIS completion, enabling patients to heal at home. Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical experience as a RN.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care to patients in accordance with the plan of care and agency policies.
• Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
• Initiates appropriate referrals when indicated.
• Promotes and maintains patient health through teaching and appropriate rehabilitative measures.
• Assists patients in learning appropriate self-care techniques.
• Other duties as assigned.
