Job summary

Amedisys is seeking an Admissions Registered Nurse for part-time daytime shifts in Charleston, SC. This role supports patients at home, performing start of care assessments and coordinating plan of care with clinical teams.

Ideal candidates hold an RN license for the assigned state(s) and have CPR certification, with at least one year of RN experience. Amedisys offers comprehensive benefits and flexible scheduling to assist patients where they are most comfortable—at home.

Qualifications

• Current RN license, state(s) to be assigned to.
• One year of clinical RN experience, or approval by VP of clinical for less than one year.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Perform start of care comprehensive assessment and plan of care development and implementation.
• Complete timely and accurate OASIS assessments.
• Provide skilled nursing care per plan and agency policies.
• Communicate with staff, physicians, families, and resources to maintain continuity of care.
• Initiate referrals when indicated and promote patient health through teaching and rehab measures.
• Assist patients in learning self-care techniques.
• Other duties as assigned.

• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care to patients in accordance with the plan of care and agency policies.
• Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
• Initiates appropriate referrals when indicated.
• Promotes and maintains patient health through teaching and appropriate rehabilitative measures.
• Assists patients in learning appropriate self-care techniques.
• Other duties as assigned.

• One year of clinical experience as a n RN. If less than one year clinical experience, you must be approved by VP of clinical.
