Job summary

Amedisys in Warwick, RI is seeking an admissions registered nurse to start-of-care patients, performing comprehensive assessments and developing plans of care. This full-time day role offers a $10,000 sign-on bonus and attractive pay.

You will complete OASIS assessments, provide skilled nursing care per the plan, and communicate with staff, physicians and families to ensure continuity of care. Reliable transportation, a valid RN license and CPR are required.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical experience as an RN. If less than one year, approval by VP of clinical.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs start-of-care comprehensive assessment and develops plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care per plan and agency policies.
• Communicates with staff, physicians, family, and community resources to ensure continuity of care.
• Initiates referrals when indicated.
• Promotes patient health through teaching and rehabilitative measures.
• Assists patients in learning self-care techniques.

• Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
• Initiates appropriate referrals when indicated.
• Promotes and maintains patient health through teaching and appropriate rehabilitative measures.
• Assists patients in learning appropriate self-care techniques.
• Other duties as assigned.
