Qualifications

• Current RN license, state(s) you are assigned to work.
• One year of clinical experience as an RN; if less than one year, approval by VP of clinical.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs start of care comprehensive assessment, evaluation, development and implementation of plan of care.
• Completes OASIS assessments timely and accurately.
• Provides skilled nursing care to patients in accordance with the plan of care and agency policies.
• Helps to achieve and maintain continuity of patient care by communicating information with staff, physicians, family members and community resources.
• Initiates appropriate referrals when indicated.
• Promotes and maintains patient health through teaching and rehabilitative measures.
• Assists patients in learning appropriate self-care techniques.
• Other duties as assigned.
