Amedisys Home Health is seeking a part-time Admissions Registered Nurse in Shelton, CT. You will begin with a start-of-care assessment and implement patient care plans, with potential to move to full-time based on needs and performance.

The role includes completing OASIS assessments, delivering skilled nursing care at patients’ homes, and coordinating with physicians and family members to ensure high-quality, continuous care.

Qualifications

• Current RN license in applicable state(s).
• < 1 year of RN clinical experience requires VP approval.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsabilités

• Perform start of care comprehensive assessment and develop/implement plan of care.
• Complete OASIS assessments timely and accurately.
• Provide skilled nursing care per plan and agency policies.
• Maintain continuity of patient care by communicating with staff, physicians, families, and resources.
• Initiate referrals when indicated and appropriate.
• Promote patient health through teaching and rehabilitative measures.
• Assist patients in learning self-care techniques.
• Handle other duties as assigned.

• Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
• Initiates appropriate referrals when indicated.
• Promotes and maintains patient health through teaching and appropriate rehabilitative measures.
• Assists patients in learning appropriate self-care techniques.
• Other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical experience as an RN. If less than one year clinical experience, you must be approved by VP of clinical.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
