Job summary

Bluegrass Care Navigators in Kentucky seeks a Home Care Manager for the PACE program to develop and implement home-based services for program participants. You will educate participants, families, and caregivers under the direction of the IDT and PACE leadership.

The role requires RN licensure, an associate degree (bachelor preferred), at least two years of nursing experience (including elderly care), supervisory experience, CPR, and a valid driver's license.

Qualifications

• RN license in good standing with state board.
• Associate degree in Nursing required; Bachelor’s degree preferred.
• Minimum two years of general nursing experience; hospice/home health/medical/surgical/CCU preferred.
• At least one year of supervisory experience.
• Experience with frail and elderly population.
• Food handling permit required (to be completed on hire).
• Current CPR certification.
• Strong written documentation and observation skills; good nursing judgment.
• Self-directed with ability to work with minimal supervision.
• Willingness to work within interdisciplinary team and flexible scheduling.
• Must have valid driver's license and insured vehicle.

Responsibilities

• Develop and implement home care services for program participants.
• Educate participants, families, and caregivers as directed by IDT, Center Director, and AVP of PACE Operations.
• Oversee clinician visits in home settings and facilities to ensure high-quality care and patient experience.
• Coordinate delivery of all home-based services, equipment, and supplies.
