Job summary

Bluegrass Care Navigators is seeking an APRN to join the PACE Care team in Kentucky. The role involves direct participant care, care planning, and collaboration with the medical director and interdisciplinary team to establish individualized plans of care for older adults.

The APRN will maintain current licensure, provide telephonic and in-person consultations, and support quality improvement initiatives within the PACE Center environment, serving Kentuckians aged 55+ in the community.

Qualifications

• Graduate of an accredited master’s degree nursing program.
• Certified as an Advanced Practice Registered Nurse (APRN) as a Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP).
• Currently licensed as an APRN in good standing with the applicable State Board of Nursing.

Responsibilities

• Provides direct participant care and evaluates participant outcomes.
• Sets and evaluates practice standards and provides consultation to nurses.
• Maintains professional knowledge and skills as an expert in the PACE Center’s advanced nursing practice.
