Job summary

Hospice of The Bluegrass, Inc. is seeking a Home Care Manager to develop and implement home care services for participants in Lexington. You will oversee clinician visits in homes and facilities, ensuring efficient, high-quality care and positive patient experiences.

You will educate participants, families and caregivers under guidance of the IDT, Center Director, and AVP of PACE Operations. Typical duties include coordinating home-based services, equipment and supplies, and supervising nursing

Qualifications

• RN licensed in Kentucky.
• Two years nursing experience; hospice/home health preferred.
• At least one year supervisory experience.
• Valid driver's license with insured vehicle.

Responsibilities

• Oversee and coordinate home visits and delivery of services.
• Educate participants, families and caregivers per IDT directions.
• Ensure high-quality care and timely documentation for patient experience.
