Job summary

Senior Community Care of Kentucky PACE, a program of Volunteers of America National Services, invites a Registered Nurse to join the Louisville, KY team. The RN provides skilled nursing care, coordinates with the Interdisciplinary Team, and supports participants living safely in the community.

The role emphasizes education for participants and families, adherence to care plans, and collaboration with primary providers to ensure quality outcomes in a compassionate, mission-driven environment.

Qualifications

Current licensure in the state of Kentucky as a Registered Nurse.
Must have a valid driver's license and have means of transportation to assist participants in their homes or other settings.
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Current CPR Certification.
A minimum of one-year (1) experience in working with the frail or elderly population.

Responsibilities

Functions as a member of the Interdisciplinary Team.
Maintains regular attendance at Interdisciplinary Team meetings and coordinates care.
Develops and implements a plan of care to meet participants' needs.
Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care.
Performs general and skilled nursing care to participants as assigned.
Implements the plan of care and physician orders in coordination with the Care/Interdisciplinary Team.
