LIFE COORDINATED Commonwealth PACE is seeking a Registered Nurse to join an interdisciplinary team delivering comprehensive home-based care under the PACE program. Responsibilities

• Gives skilled nursing care and prescribed treatments to participants in homes or at the adult day health center.
• Conducts comprehensive assessments to identify healthcare needs of elderly participants.
• Manages the medication system and coordinates care with IDT members.
• Teaches classes and coordinates referrals to other services as needed.
• Maintains timely documentation and supports quality improvement initiatives.
