Job summary

Bluegrass Care Navigators is seeking a PACE Home Care CNA to support all-inclusive care for elderly participants in their homes and the PACE Center in Lexington. You will assist with daily living activities, monitor health, and coordinate with the interdisciplinary team to maintain safety and wellbeing.

The role requires a valid Kentucky CNA license, CPR, and a compassionate, self-directed approach to care with flexibility for PRN scheduling.

Qualifications

Must be at least 18 years old with a High School Diploma or GED.
State Registered Nurse Aide in Kentucky and completion of a recognized CNA training program.
At least 1 year of experience with frail and elderly, or proper training on hire.
Food handling permit to be completed upon hire and CPR certification current.
Ability to read/interpret safety and procedure documents and write routine reports.
Must have a valid driver’s license with insured vehicle for participant transportation.

Responsibilities

Provide essential personal care (bathing, dressing, toileting) and daily living assistance.
Assist with meal preparation and feeding; monitor vital signs; document intake and output.
Maintain a clean, safe home/center environment; perform routine housekeeping and chore tasks.
Transport and assist participants with simple errands; collaborate with Interdisciplinary Team.
Report changes in participant condition to clinical staff and follow emergency procedures.
