Job summary

Bluegrass Care Navigators in Lexington, KY seeks a dedicated PACE Activities Assistant to support planning and delivering therapeutic, social, and recreational activities for older adults in the PACE program. You will collaborate with the IDT to promote wellbeing, independence, and individualized care in a safe, inclusive environment.

Qualifications include a high school diploma or equivalent, prior experience with older adults or in healthcare settings, and a willingness to obtain CPR/First Aid

Qualifications

• Experience working with older adults or in healthcare settings is preferred.
• CPR/First Aid certification or willingness to obtain it.
• Must have a valid driver’s license and reliable vehicle.

Responsibilities

• Plan and coordinate therapeutic, social, and recreational activities for PACE participants.
• Collaborate with the Interdisciplinary Team to promote well-being and independence in a safe, inclusive setting.
