Job summary

Bluegrass Care Navigators’ PACE program seeks a Center CNA to provide direct personal care and supportive services to participants at the PACE Center in Kentucky. Under RN supervision, you assist with activities of daily living, promote safety and independence, and help with center-based activities and transportation as needed.

The role requires a high school diploma or GED, a Kentucky SRNA license, a CNA training program, and at least one year of experience with frail and elderly populations.

Qualifications

• High school diploma or GED required; must be at least 18.
• Current SRNA certification through Kentucky Board of Nursing.
• Completion of an approved CNA training program.
• Minimum of one year of experience with frail and elderly population.
• Food handling permit required.
• Maintains CPR certification.
• Ability to work independently with minimal supervision.
• Strong communication and compassionate care for adults and elderly.
• Must be a licensed driver.

Responsibilities

• Provide direct personal care and supportive services at the PACE Center.
• Assist with activities of daily living and promote safety and independence.
• Observe participant changes and report to RN.
• Ensure accurate documentation in compliance with regulatory requirements.
• Assist with participant transportation as needed.
