Advocate Health at Home is seeking a Registered Nurse Home Care Liaison for an onsite role at Lutheran General Hospital in Park Ridge, IL. This full-time position requires coordinating care, managing referrals, and ensuring seamless transitions from hospital to home.

You will educate care teams on Medicare guidelines and home care services, verify benefits, and support documentation for reimbursement while collaborating with multidisciplinary teams to improve patient outcomes.

Qualifications

• Current Registered Nurse (RN) license.
• 3-4 years of nursing experience.
• Strong understanding of Medicare regulations and home care eligibility.
• Exceptional communication and customer service skills.
• Ability to manage multiple priorities and navigate multiple computer systems.

Responsabilités

• Evaluate and process referrals for Home Health, Pediatrics, and Infusion Therapy services.
• Assess patients for eligibility and appropriateness for home care based on clinical and regulatory guidelines.
• Collaborate with hospital care management, social workers, and physicians to support discharge planning and transitions of care.
• Initiate interdisciplinary communication and document the initial home care plan.
