Job summary

Advocate Health in Park Ridge is expanding its Hospital at Home program and seeks a skilled Registered Nurse to deliver hospital-level care in patients’ homes. This full-time 12-hour day shift role. A $5,000 sign-on bonus is available to external hires.

You’ll work with a collaborative interdisciplinary team, manage 3–4 patient assignments, utilize remote monitoring tools, and coordinate discharge planning to ensure safe, high-quality outcomes.

Qualifications

• Active RN license in the state of Illinois (IL).
• BLS certification (AHA) required.
• Valid driver’s license with a clean driving record.
• 2+ years of recent acute inpatient care or ED experience preferred.
• Strong IV starts, phlebotomy, and clinical assessment skills.
• Ability to work independently in home settings and manage care remotely.
• Excellent communication and critical thinking abilities.
• Comfortable with technology and remote monitoring tools.

Responsibilities

• Deliver acute, hospital-level care in patients’ homes.
