Advocate Health is seeking a Registered Nurse (RN) for Mobile In-Home RN responsibilities in the Park Ridge, IL area. The role combines home health autonomy with inpatient nursing complexity, delivering hospital-level care in patients' homes within a defined radius of Lutheran General Hospital.

The position requires a valid RN license, BLS certification, two years of acute bedside or emergency nursing experience, and the ability to travel locally.

Qualificações

• Completion of an accredited or approved nursing program.
• Current RN license in applicable state.
• BLS for Healthcare Providers from AHA.
• Valid driver's license and acceptable driving record.
• Ability to travel within designated geographic area.

Responsabilidades

• Assess patients using the nursing process in home setting.
• Plan and implement individualized home care interventions.
• Collaborate with virtual care team to coordinate patient care.
• Lead in-home assessments and adjust plans as needed.
