Candidates should have a valid RN license, clinical knowledge in home health, and strong communication skills, along with the ability to manage patient assessments effectively.

Qualifications

• Valid RN license in Pennsylvania required.
• Preferred 3+ years of clinical coordination experience.
• Must have clinical knowledge common to Home Health and Hospice.

Responsibilities

• Generate referrals for home health, hospice, palliative care.
• Assess patient needs for smooth discharge transitions.
• Build relationships with case managers and social workers.
