Job summary

Bristol Hospital and Health Care is seeking a Home Care Services Liaison to build relationships with community referral sources and ensure smooth transitions for patients onto home care and hospice services.

The role is primarily field-based with travel within the service area, requires an active RN license, 1–2 years of clinical nursing experience, and familiarity with Medicare/Medicaid regulations; EMR/EHR proficiency is expected to support ongoing care coordination.

Qualifications

• Active, unencumbered RN license in the state of practice.
• Minimum 1–2 years of clinical nursing experience.
• Experience in home health, hospice, case management, or intake coordination preferred.
• Knowledge of Medicare/Medicaid regulations for home care and hospice.
• Strong assessment, organizational, and communication skills.
• Proficiency with electronic medical records (EMR/EHR) systems.

Responsibilities

• Build and maintain relationships with hospitals, medical practices, SNFs, ALFs and other referral sources.
• Educate referral sources on available home care and hospice services and admission criteria.
• Generate referrals through ongoing outreach, networking, and relationship management.
• Conduct regular in-person visits and follow-ups with referral sources.
• Collaborate with intake and home care/hospice leadership to ensure smooth patient transitions and admission to services.
• Represent the agency at community events, health fairs, and meetings.
• Stay informed on industry trends, regulatory changes, and competitor activity.
