Job summary

Residential Home Health and Residential Hospice in Northbrook, IL is seeking a compassionate Home Health Transitional Nurse Liaison to partner with our field staff to support patients we serve. The RN must be licensed in good standing and have at least one year in acute care plus one year in a customer service capacity.

Responsibilities include introducing our services, coordinating start of care visits, and educating patients and families.

Qualifications

• RN licensure in good standing.
