Job summary

WellMed, part of the Optum family, is seeking a Healthcare Manager to support high-risk patients and coordinate care across multidisciplinary teams. The role requires RN licensure, BLS, and experience in a clinical setting.

Strong EMR skills and excellent communication are essential. Responsibilities include coordinating care plans, ensuring documentation quality, and facilitating transitions from hospital to home while maintaining patient education and advocacy.

Qualifications

• Active RN license in state of employment
• Bachelor's or Associate degree in Nursing with 4+ years of RN experience
• Current BLS certification
• 2+ years experience in physician's office, clinical or hospital setting
• Knowledge of COPD/asthma, diabetes, CHF and IHD
• Proficient with electronic medical records (EMR)

Responsibilities

• Identify patients at high risk in collaboration with providers and clinic staff
• Support longitudinal care and chronic condition management
• Perform medication reconciliation and patient education
• Coordinate transitions from inpatient to outpatient care
• Document accurately in the electronic medical record
• Participate in daily huddles and PCC meetings and reporting
