Qualifications

• ADN or BSN degree from an accredited program.
• Active RN license in good standing.
• Current BLS/CPR certification.
• Experience in long-term, post-acute, rehabilitative or geriatric nursing preferred.
• English proficiency in reading, writing, and speaking.

Responsibilities

• Perform comprehensive clinical assessments and develop care plans.
• Administer medications and treatments; monitor patient status.
• Coach and supervise LPNs and CNAs; delegate care as appropriate.
• Collaborate with physicians and interdisciplinary teams; ensure safe discharge planning.
• Maintain accurate PCC documentation and comply with HIPAA.
