Qualifications

• Graduate of an accredited School of Nursing (ADN or BSN).
• Active RN license in the state of practice.
• Current BLS/CPR certification.
• Experience in long-term care, post-acute, or geriatric nursing preferred.
• English proficiency for clear clinical documentation.
• Must pass health screenings including TB testing.

Responsibilities

• Perform comprehensive clinical assessments and develop individualized care plans.
• Administer medications and treatments while monitoring patient responses.
• Collaborate with physicians and interdisciplinary teams for safe discharge planning.
• Lead and mentor LPNs and CNAs; delegate responsibilities appropriately.
• Maintain accurate, real-time documentation in PCC EHR.
• Ensure compliance with HIPAA, infection control, and safety guidelines.
