Job summary

Provident Home Healthcare is seeking an RN Group Site Lead to provide hands-on, in-person care at multiple Minnesota locations. You will contribute to assessments, care plans, EMR documentation, and collaborate with a dedicated team to support high-acuity clients.

This full-time role includes weekly case-management training and on-site duties. Ideal candidates have at least 6 months of RN experience, current CPR, and an active RN license.

Qualifications

• Six months experience as an RN providing direct patient care.
• Current CPR certification.
• Documentation of negative TB test (Mantoux/blood test/chest x-ray).
• Current MN driver’s license and reliable transportation.
• Proficiency with EMR software or ability to learn quickly.

Responsibilities

• Provide hands-on, in-person care to high-acuity clients at multiple locations.
• Receive weekly training on case management, assessments, and care planning.
• Collaborate with other care providers, manage supplies, and document in EMR.
