Job summary

Provident Home Healthcare is seeking an RN Group Site Lead to provide hands-on, in-person care at multiple Minnesota locations. You will contribute to assessments, care plans, EMR documentation, and collaborate with a dedicated team to support high-acuity clients.

This full-time role includes weekly case-management training and on-site duties. Ideal candidates have at least 6 months of RN experience, current CPR, and an active RN license.

Qualifications

• Six months experience as an RN providing direct patient care.
• Current CPR certification.
• Documentation of negative TB test (Mantoux/blood test/chest x-ray).
• Current MN driver’s license and reliable transportation.
• Proficiency with EMR software or ability to learn quickly.

Responsibilities

• Provide hands-on, in-person care to high-acuity clients at multiple locations.
• Receive weekly training on case management, assessments, and care planning.
• Collaborate with other care providers, manage supplies, and document in EMR.

Role and Responsibilities

This position is a dynamic and hands-on, rewarding position effecting positive and life-giving care to 3–4 high-acuity individuals.

In addition to hands-on care, this role will train on the basics of case management in a supportive environment, with one designated day per week devoted to instruction, collaboration and practice.

You will receive training in assessments, care plans, EMR documentation, collaborating with other care providers for the client, supply management, communications and team building.

RN Group Site Lead Position

• Location: St. Paul,Falcon Heights, Maplewood, Roseville and St. Anthony

• Schedule: Full Time:32 hours or more per week

• Shift: 8:00 am to 4 or5:00pm with some flexibility

• On-Call Rotation: RN Group Site Leads may participate in an on-call rotation if desired. On-call is paid separately, over and above the regular pay.
  Requirements

• Position is **IN-PERSON ONLY (NOT REMOTE)**providing meaningful, hands-on care

• Six (6) months experience as an RN providing direct patient care

• Must be eligible to work in the United States

• Current CPR Certification

• Documentation of negative TB test (Mantoux/blood test/chest x-ray) prior to or upon hire

• Current MN Driver’s License and reliable transportation

• Excellent organization, planning and communication skills

• Proficiency with Electronic Medical Records (EMR) software or the ability and enthusiasm to quickly learn

• A positive, team-oriented mindsetand a genuine passion for improving the lives of individuals with complex medical needs
