Job summary

Provident Home Healthcare seeks an RN Group Site Lead to provide hands-on, in-person nursing care to high-acuity clients in multiple Saint Paul area sites. You will train in basic case management and collaborate with the care team, with one day weekly dedicated to instruction and practice.

Ideal candidates have at least 6 months RN experience, CPR certification, and a valid Minnesota driver’s license. This role offers on-site work with schedule flexibility and opportunities for advancement.

Qualifications

• Six months experience as an RN providing direct patient care.
• Current CPR Certification.
• Documentation of negative TB test (Mantoux/blood test/chest x-ray) prior to or upon hire.
• Current MN Driver's License and reliable transportation.

Responsibilities

• Provide hands-on, in-person RN care to high-acuity clients.
• Participate in training on case management with one designated day per week.
• Assist with assessments, care plans, EMR documentation, and coordination with other care providers.
