Qualifications

Responsibilities

• Assess patients and collaborate with the healthcare team
• Develop personalized care plans with patients and families
• Deliver evidence-based patient care and educate on treatments
• Evaluate outcomes and adjust care based on data
• Drive improvement with Clinical Manager to support better patient outcomes
• Promote High Reliability in care and operations
• Lead and mentor peers while managing patient care
• Document the nursing process in the EMR
• Maintain compliance with policies and trainings
