Job summary

Activate Care in Massachusetts is seeking an experienced RN Supervisor to lead a distributed team of RN Assessors delivering in‑home health assessments for dually eligible members. This player‑coach role demands quality oversight, timely audits, and development of staff while coordinating with the health plan’s care management team.

You will supervise field staff, ensure adherence to HIPAA and NCQA standards, manage caseloads and schedules, and drive performance metrics in a hybrid field setting

Qualifications

• Active RN license in good standing in Massachusetts.
• BSN required.
• Minimum of 3 years RN experience, including home health or community assessments.
• Supervisory or mentorship experience.
• Strong QA/audit skills and coaching ability.
• Experience with HIPAA and NCQA standards is preferred.

Responsibilities

• Lead and develop RN Assessors with hiring, onboarding, coaching, and retention.
• Provide supervision, field observations, and ride-alongs.
• Maintain coverage plans to protect timeliness during PTO and turnover.
• Audit assessments pre-submission for accuracy and completeness.
• Ensure documentation in GuidingCare within 48 hours.
• Manage caseload distribution, scheduling, and territory planning.
