Job summary

Activate Care is seeking a field-based Registered Nurse to conduct in-home health assessments for dually eligible members across Massachusetts. You will document findings in GuidingCare and hand off to the care management team, ensuring timely, accurate reporting and adherence to privacy standards.

The role requires MA RN licensure, at least 1 year in home health, and the ability to travel to members' homes. Flu vaccination is expected and ongoing training will be provided.

Qualifications

• Active Registered Nurse (RN) license in Massachusetts in good standing.
• Minimum 1 year of clinical experience in home health.
• Must reside within 30 minutes of Gardner, MA.

Responsibilities

• Travel to members' homes across Massachusetts to perform in-person health assessments.
• Document assessments in GuidingCare and provide handoff to care management.
• Submit assessments within 48 hours with high completeness and accuracy.
• Report abuse or exploitation of vulnerable adults as a mandated reporter.
• Maintain licensure and pursue required ongoing training.
