Job summary

Activate Care is seeking a Field Registered Nurse (RN) to perform in-home health assessments for dually eligible members across Massachusetts. The role involves state-wide travel and documenting results in the care management platform.

The position requires MA licensure, an nursing degree, and a valid driver’s license with a personal vehicle. This is a full-time, field-based role with strict documentation timelines and HIPAA compliance.

Qualifications

• Active RN license in MA in good standing.
• AS degree in nursing; BSN preferred.
• Valid driver’s license, personal vehicle, insurance.
• Minimum 1 year in home health; 1 year assessments preferred.
• Experience with dual-eligible populations or managed care preferred.

Responsibilities

• Travel to members' homes across MA to complete assessments.
• Document assessments as per workflows; submit SOAP notes.
• Maintain high-quality, timely documentation within 48 hours.
• Adhere to HIPAA, safety, and regulatory standards.
• Engage with members and families with cultural and linguistic sensitivity.
• Routinely travel and manage a flexible scheduling of visits.
