    Humana Healthy Horizons in Virginia seeks an RN, Field Care Manager Nurse 2 (Maternal-Health) to assess and evaluate high-acuity members' needs to promote optimal wellness. The role involves guiding members and families, facilitating resource interaction, and working independently to determine appropriate care actions.
    • Conduct health risk assessments across the care continuum.
    • Create individualized care plans and participate in care team meetings.
    • Utilize strategies to manage physical, environmental, and psycho-social health issues.
    • Identify and resolve barriers to effective care.
    • Monitor members' progress towards health outcomes.
    • Understand departmental and organizational strategies and objectives.
    • Make independent decisions within the scope of work, sometimes in ambiguous situations.
    • Collaborate with providers and departments to meet member needs.
    • Ensure compliance with contractual and regulatory standards.
    • Follow established guidelines and procedures.

Required Qualifications

  • Must reside in the Southwest Region of Virginia (counties/cities including Lee, Scott, Washington, Bristol, Russell, Norton, Wise, Buchanan, Dickenson, Tazewell, Smyth, Bland, Grayson, Gala, Wythe, Carroll or within 20 miles of bordering states TN, KY).
  • Active RN license in Virginia with no disciplinary action.
  • At least 2 years of healthcare or case management experience.
  • Minimum 1 year of acute care experience in obstetrics, women’s care, labor/delivery, NICU, or clinical triage.
  • Proficient in Microsoft Office applications.
  • Knowledge of community health and social services.
  • Strong communication and interpersonal skills.
  • Ability to work independently within scope.
  • Experience with electronic medical records and web navigation.
  • Must be able to travel regionally for face-to-face assessments.
  • Must have a valid driver’s license and vehicle insurance.

Preferred Qualifications

  • Bachelor's Degree in Nursing (BSN).
  • 3-5 years in-home assessment and care coordination.
  • Lactation certification or experience.
  • Experience in health promotion and wellness.
  • Managed care experience.
  • Certification in Case Management (CCM).
  • Motivational Interviewing certification.
  • Bilingual abilities, especially in English/Spanish, Arabic, Vietnamese, Amharic, Urdu, or others.

Additional Information

  • This is a remote role requiring travel.
  • Travel estimated at 50-75% to meet members, families, and providers.
  • Work hours: Monday to Friday, 8:00 am – 5:00 pm EST.
  • Language proficiency assessment required for non-English speakers.

Internet Requirements

  • Minimum internet speed: 25 Mbps download, 10 Mbps upload.
  • Use of satellite or cellular internet requires approval.
  • Work from a dedicated, interruption-free space.

Interview Process

Use of HireVue technology for interviews.

Scheduled weekly hours: 40

Salary range: $71,100 - $97,800 annually, with potential bonuses based on performance.
