Humana Healthy Horizons in Virginia is seeking an RN, Field Care Manager Nurse 2 (Maternal-Health). The role involves assessing and evaluating high-acuity members' needs to promote wellness by guiding members and families toward appropriate resources, especially for pregnant and postpartum individuals. The work is varied and often requires independent judgment.

  • Conduct health risk assessments across the care continuum.
  • Create individualized care plans and participate in interdisciplinary team meetings.
  • Use diverse strategies to manage physical, environmental, and psychosocial health issues.
  • Identify and resolve barriers to effective care.
  • Monitor and evaluate members' progress towards health outcomes.
  • Understand and align with departmental and organizational strategies.
  • Make independent decisions regarding work methods, occasionally in ambiguous situations.
  • Collaborate with internal and external providers to meet member needs.
  • Ensure compliance with contractual and regulatory standards.
  • Follow established guidelines and procedures.

Required Qualifications

  • Must reside in the Southwest Region of Virginia, within specific counties or within a 20-mile radius of the border with Tennessee or Kentucky.
  • Active RN license in Virginia without disciplinary actions.
  • At least 2 years of healthcare and/or case management experience.
  • Minimum 1 year of acute care experience in obstetrics, women's health, labor and delivery, NICU, or clinical triage.
  • Proficient in Microsoft Word, Outlook, and Excel.
  • Knowledge of community health and social services.
  • Excellent communication and interpersonal skills.
  • Ability to work independently within scope.
  • Experience with electronic medical records and web navigation.
  • Must pass TB screening and have a valid driver’s license with insurance for travel.

Preferred Qualifications

  • Bachelor's Degree in Nursing (BSN).
  • 3-5 years in-home assessment and care coordination.
  • Lactation certification, preferably as a lactation consultant.
  • Experience in health promotion, coaching, wellness, and managed care.
  • Certification in Case Management (CCM).
  • Motivational Interviewing certification or knowledge.
  • Bilingual in English and another language such as Spanish, Arabic, Vietnamese, Amharic, or Urdu.

Additional Information

  • This is a remote role requiring travel to members' homes and facilities, approximately 50-75% of the time.
  • Work hours are Monday–Friday, 8:00 am – 5:00 pm EST.
  • Must meet internet requirements for remote work and work from a dedicated, interruption-free space.
  • Interview process may include HireVue assessments.

Compensation & Benefits

Starting salary ranges from $71,100 to $97,800 annually, with potential bonuses based on performance. Benefits include medical, dental, vision, 401(k), paid time off, disability, life insurance, and wellness programs.
