RN, Field Care Manager - Southwest Region, Virginia

Humana Healthy Horizons in Virginia is seeking an RN, Field Care Manager Nurse 2 (Maternal-Health) who will assess and evaluate high-acuity members' needs to promote wellness by guiding members and families toward appropriate resources, especially for pregnant and postpartum members. The role involves varied work assignments requiring interpretation and independent decision-making.

  • Conduct health risk assessments across the care continuum.
  • Create individualized care plans and participate in interdisciplinary team meetings.
  • Employ strategies to manage physical, environmental, and psycho-social health issues.
  • Identify and resolve barriers to effective care.
  • Monitor members' progress toward health outcomes through assessments and evaluations.
  • Understand departmental and organizational strategies and objectives.
  • Make independent work decisions within scope, occasionally in ambiguous situations.
  • Collaborate with internal and external providers to meet members' needs.
  • Ensure compliance with contractual and regulatory standards.
  • Follow established guidelines and procedures.

Required Qualifications

  • Reside in Southwest Virginia, specifically in Lee, Scott, Washington, Bristol, Russell, Norton, Wise, Buchanan, Dickenson, Tazewell, Smyth, Bland, Grayson, Gala, Wythe, Carroll counties or within 20 miles of these areas in a bordering state (TN, KY).
  • Active RN license in Virginia without disciplinary actions.
  • At least 2 years of healthcare or case management experience.
  • Minimum 1 year of acute care experience in obstetrics, women's care, labor and delivery, NICU, or clinical triage.
  • Proficient in Microsoft Word, Outlook, and Excel.
  • Knowledge of community health and social services.
  • Excellent communication and interpersonal skills to build rapport quickly.
  • Ability to work independently within role scope.
  • Experience with electronic medical records and web navigation.
  • Willingness to undergo TB screening and travel within the region.
  • Valid driver's license and personal vehicle insurance meeting state minimums.

Preferred Qualifications

  • Bachelor's in Nursing (BSN).
  • 3-5 years in-home assessment and care coordination.
  • Certified Lactation Consultant or lactation experience.
  • Experience in health promotion, coaching, or wellness.
  • Managed care experience.
  • Case Management Certification (CCM).
  • Motivational Interviewing certification or knowledge.
  • Bilingual in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or other languages.

Additional Information

  • This is a remote role requiring travel to meet members and providers.
  • Travel is estimated at 50-75% within the region.
  • Work hours are Monday to Friday, 8:00 am to 5:00 pm EST.
  • Must meet internet requirements for remote work and work from a dedicated, interruption-free space.

Compensation and Benefits
