RN, Field Care Manager - Southwest Region, Virginia

Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) to assess and evaluate high-acuity members' needs and requirements to achieve and/or maintain optimal wellness. The role involves guiding members and families toward appropriate resources for the care of pregnant and postpartum members. The assignments are varied and often require interpretation and independent decision-making.
- Responsible for health risk assessments across the continuum of care.
- Creates individualized care plans and attends interdisciplinary team meetings.
- Employs strategies to manage physical, environmental, and psycho-social health issues.
- Identifies and resolves barriers to effective care.
- Monitors member progress toward desired outcomes.
- Understand departmental and organizational strategies and objectives.
- Makes decisions regarding work methods, often in ambiguous situations, with minimal guidance.
- Collaborates with internal and external providers to meet member needs.
- Ensures compliance with contractual and regulatory standards.
- Follows established guidelines and procedures.

Required Qualifications
- Must reside in the Southwest Region of Virginia, within specified counties or within a 20-mile radius of the region in a bordering state (TN, KY).
- Active RN license in Virginia without disciplinary action.
- At least 2 years of healthcare or case management experience.
- Minimum 1 year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
- Intermediate to advanced skills with Microsoft Office Suite.
- Knowledge of community health and social service agencies.
- Excellent communication and interpersonal skills.
- Ability to work independently within the role's scope.
- Experience with electronic medical records and web navigation.
- This role involves patient contact and TB screening; screening will be required.
- Must be able to travel regionally and have a valid driver’s license with personal vehicle insurance.
Preferred Qualifications
- Bachelor's of Science in Nursing (BSN).
- 3-5 years of in-home assessment and care coordination.
- Lactation certification, preferably as a lactation consultant.
- Experience in health promotion, coaching, and wellness.
- Managed care experience.
- Case Management Certification (CCM).
- Motivational Interviewing certification or knowledge.
- Bilingual abilities in English and another language such as Spanish, Arabic, Vietnamese, Amharic, or Urdu.
Additional Information
- This is a remote position requiring travel.
- Travel is estimated at 50-75% within the region, with occasional onsite meetings in Glen Allen, VA.
- Work hours are Monday to Friday, 8:00 am to 5:00 pm EST.
- Applicants must pass a language proficiency assessment if speaking with members in a language other than English.
Technical Requirements
- Home internet must meet minimum speed requirements (25 Mbps download, 10 Mbps upload).
- Work must be conducted from a dedicated, interruption-free space.

Compensation & Benefits
Starting salary ranges from $71,100 to $97,800 annually, with potential bonuses based on performance. Benefits include health insurance, 401(k), paid time off, disability, life insurance, and more.
