RN, Field Care Manager - Southwest Region, Virginia

Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) who will assess and evaluate high acuity member's needs and requirements to achieve and/or maintain optimal wellness by guiding members and families toward appropriate resources for the care of pregnant and postpartum members. The RN, Field Care Manager Nurse 2 (Maternal-Health) work assignments are varied and often require interpretation and independent decision-making.
- Responsible for health risk assessments along the care continuum for members.
- Creates individualized care plans, assesses members, and participates in interdisciplinary care team meetings.
- Employs various strategies and techniques to manage physical, environmental, and psycho-social health issues.
- Identifies and resolves barriers to effective care.
- Monitors members' progress towards desired outcomes through ongoing assessments and evaluations.
- Understand department, segment, and organizational strategies and objectives, including their linkages to related areas.
- Makes independent decisions regarding work methods, occasionally in ambiguous situations, requiring minimal guidance.
- Collaborates with internal and external providers to meet member needs.
- Ensures compliance with contractual and regulatory requirements.
- Follows established guidelines and procedures.

Required Qualifications
- Reside in the Southwest Region of Virginia, within the counties/cities of Lee, Scott, Washington, Bristol, Russell, Norton, Wise, Buchanan, Dickenson, Tazewell, Smyth, Bland, Grayson, Gala, Wythe, Carroll, or within a 20-mile radius of the region in bordering states (TN, KY).
- Active RN license in Virginia without disciplinary action.
- At least 2 years of healthcare and/or case management experience.
- Minimum 1 year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
- Proficient with Microsoft Word, Outlook, and Excel.
- Knowledge of community health and social services.
- Excellent communication and interpersonal skills.
- Ability to work independently within the role and scope.
- Experience with electronic medical records and other software applications.
- Ability to travel regionally for face-to-face assessments.
- Possession of a valid driver’s license and personal vehicle insurance.
- This role involves TB screening and participation in Humana’s driver safety program.
Preferred Qualifications
- Bachelor’s of Science in Nursing (BSN).
- 3-5 years of in-home assessment and care coordination.
- Lactation certification or experience.
- Experience with health promotion and wellness programs.
- Previous managed care experience.
- Certification in Case Management (CCM).
- Motivational Interviewing certification.
- Bilingual in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or other languages.
Additional Information
- This is a remote position requiring travel.
- Travel is approximately 50-75% to meet with members, families, and providers, with occasional onsite meetings in Glen Allen, VA.
- Work hours are Monday to Friday, 8:00 am – 5:00 pm EST.
- Applicants must pass a language proficiency assessment if speaking with members in languages other than English.
- Internet requirements: minimum 25 Mbps download and 10 Mbps upload speeds, with a dedicated workspace.

- Salary range: $71,100 - $97,800 annually, with potential bonuses based on performance.
