RN, Field Care Manager - Southwest Region, Virginia

Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) who will assess and evaluate high acuity member's needs and requirements to achieve and/or maintain optimal wellness by guiding members and families toward appropriate resources for the care of pregnant and postpartum members. The role involves varied work assignments that often require interpretation and independent decision-making.

  • Responsible for health risk assessments across the care continuum for members.
  • Creates individualized care plans and participates in interdisciplinary care team meetings.
  • Employs strategies to manage physical, environmental, and psycho-social health issues.
  • Identifies and resolves barriers to effective care.
  • Monitors members’ progress towards desired outcomes through assessments and evaluations.
  • Understand department, segment, and organizational strategies and their linkages.
  • Makes work method decisions, sometimes in ambiguous situations, with minimal supervision.
  • Collaborates with internal and external providers to meet member needs.
  • Ensures compliance with contractual and regulatory requirements.
  • Follows established guidelines and procedures.

Required Qualifications

  • Must reside in the Southwest Region of Virginia, within specified counties or within a 20-mile radius of the region in a bordering state (TN, KY).
  • Active RN license in Virginia without disciplinary action.
  • At least 2 years of experience in healthcare and/or case management.
  • Minimum 1 year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
  • Intermediate to advanced skills in Microsoft Word, Outlook, and Excel.
  • Knowledge of community health and social services.
  • Excellent communication and interpersonal skills.
  • Ability to work independently within scope.
  • Experience with electronic medical records and web navigation.
  • Ability to travel regionally for face-to-face assessments.
  • Must have a valid driver’s license and personal vehicle insurance.

Preferred Qualifications

  • Bachelor’s of Science in Nursing (BSN).
  • 3-5 years of in-home assessment and care coordination.
  • Lactation certification, preferably as a lactation consultant.
  • Experience in health promotion and wellness coaching.
  • Previous managed care experience.
  • Certification in Case Management (CCM).
  • Motivational Interviewing certification or knowledge.
  • Bilingual in English and Spanish, Arabic, Vietnamese, Amharic, or Urdu (fluency in both languages required).
Additional Information
  • This is a remote role requiring travel.
  • Travel is approximately 50-75% within the region, with occasional onsite meetings in Glen Allen, VA.
  • Work hours are Monday to Friday, 8:00 am – 5:00 pm EST.
  • Must pass a language proficiency assessment if speaking with members in a language other than English.
  • Home/Hybrid employees must meet internet speed requirements and work from a dedicated, interruption-free space.
  • Interview process involves HireVue technology.
Compensation and Benefits

Starting salary ranges from $71,100 to $97,800 annually, with potential bonuses based on performance. Humana offers comprehensive benefits including health, dental, vision, retirement plans, paid time off, disability, and life insurance.
