RN, Field Care Manager - Southwest Region, Virginia

Humana Healthy Horizons in Virginia is seeking an RN, Field Care Manager Nurse 2 (Maternal-Health) who will assess and evaluate high-acuity members' needs to achieve and/or maintain optimal wellness by guiding members and families toward appropriate resources for the care of pregnant and postpartum members. The work involves varied assignments requiring interpretation and independent decision-making.
- Responsible for health risk assessments along the care continuum.
- Creates individualized care plans and participates in interdisciplinary team meetings.
- Employs strategies to manage physical, environmental, and psycho-social health issues.
- Identifies and addresses barriers to effective care.
- Monitors member progress towards health outcomes.
- Understands departmental and organizational strategies and objectives.
- Makes autonomous decisions regarding work methods, with guidance as needed.
- Collaborates with providers and internal departments to meet member needs.
- Ensures compliance with contractual and regulatory standards.
- Follows established guidelines and procedures.

Required Qualifications
- Reside in the Southwest Region of Virginia, within specified counties or within a 20-mile radius from bordering states (TN, KY).
- Active RN license in Virginia without disciplinary action.
- At least 2 years of healthcare or case management experience.
- Minimum 1 year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
- Intermediate to advanced skills in Microsoft Office.
- Knowledge of community health and social services.
- Excellent communication and interpersonal skills.
- Ability to work independently within scope.
- Experience with electronic medical records and web navigation.
- Must pass TB screening and have a valid driver’s license with insurance.
- Ability to travel regionally for face-to-face assessments.
Preferred Qualifications
- Bachelor’s of Science in Nursing (BSN).
- 3-5 years of in-home assessment and care coordination.
- Lactation certification or experience.
- Experience in health promotion, coaching, wellness, or managed care.
- Certification in Case Management (CCM).
- Motivational Interviewing certification.
- Bilingual abilities, particularly in Spanish, Arabic, Vietnamese, Amharic, Urdu, or others.
Additional Information
- This is a remote position requiring travel (50-75%).
- Work hours: Monday–Friday, 8 am–5 pm EST.
- Position involves travel to member homes and facilities.
- Applicants will undergo language proficiency testing if applicable.
- Internet requirements: minimum 25 Mbps download and 10 Mbps upload speeds.
- Interview via HireVue technology.
- Salary range: $71,100 - $97,800 annually, plus potential bonuses.
