Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) to assess and evaluate high-acuity members' needs, guiding them and their families toward resources that promote wellness, especially for members who are pregnant or postpartum. The role involves varied work assignments requiring interpretation and independent decision-making.

  • Conduct health risk assessments across the care continuum for members.
  • Create individualized care plans and participate in interdisciplinary care team meetings.
  • Employ strategies to manage physical, environmental, and psycho-social health issues.
  • Identify and resolve barriers to effective care.
  • Monitor member progress toward health outcomes through ongoing assessments.
  • Understand departmental and organizational strategies and objectives.
  • Make work-related decisions independently, with guidance as needed.
  • Collaborate with internal and external providers to meet member needs.
  • Ensure compliance with contractual and regulatory requirements.
  • Follow established guidelines and procedures.

Required Qualifications

  • Reside in the Southwest Virginia region, including counties such as Lee, Scott, Washington, Bristol, Russell, Norton, Wise, Buchanan, Dickenson, Tazewell, Smyth, Bland, Grayson, Gala, Wythe, Carroll, or within a 20-mile radius of bordering states (TN, KY).
  • Active RN license in Virginia without disciplinary actions.
  • At least 2 years of healthcare and/or case management experience.
  • Minimum 1 year of acute care experience in obstetrics, women’s health, labor and delivery, NICU, or clinical triage.
  • Proficient in Microsoft Word, Outlook, and Excel.
  • Knowledge of community health and social service agencies.
  • Excellent communication and interpersonal skills.
  • Ability to work independently within the role's scope.
  • Experience with electronic medical records and web navigation.
  • Must pass TB screening and have a valid driver’s license with insurance for travel.

Preferred Qualifications

  • Bachelor's in Nursing (BSN).
  • 3-5 years in-home assessment and care coordination.
  • Certified lactation consultant or lactation experience.
  • Experience in health promotion, coaching, and wellness.
  • Managed care experience.
  • Case Management Certification (CCM).
  • Knowledge of Motivational Interviewing.
  • Bilingual in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or others.
Additional Information
  • This is a remote role requiring travel (50-75%) within the region.
  • Work hours are Monday–Friday, 8:00 am–5:00 pm EST.
  • Applicants must pass a language proficiency assessment if speaking a language other than English.

  • Reliable internet connection (minimum 25 Mbps download, 10 Mbps upload) required.
  • Work from a dedicated, interruption-free space to protect patient information.

Compensation and Benefits

Salary ranges from $71,100 to $97,800 annually, with bonus potential based on performance. Benefits include health insurance, 401(k), paid time off, disability, life insurance, and wellness programs.
